I think you lose awareness of what you are doing gradually, and you are aware of what is happening to you. It can be exceedingly frustrating and painful.
I had a relative who gradually lost it, and he would regularly ask anyone who would listen to help him kill himself. He didn't want to live that way.
OTOH, that requires some degree of self-awareness. Trump is already so delutional, that he may continue to believe he is the smartest man in the world, even when he can't dress himself.
